Important Information for Patients enrolled with CHPW:
We will no longer be contracted with Community Health Plan of WA as of November 1, 2011.
Effective immediately we will NOT be accepting any new patients that are enrolled with CHPW.

What does this mean for our patients that have CHPW?

If you are currently enrolled with CHPW, this means that upon termination of our contract Pullman Family Medicine will no longer be a “contracted provider”. Therefore services received after the termination date will not be covered by CHPW.

If you are enrolled in the Washington State Medicaid Program, Healthy Options, or Basic Health program, you do have the option to request a different insurance plan. A list of insurance plans can be obtained at the following websites. If you prefer, you can complete the request to change your insurance plan and we can submit it for you. Please check with a member of our office staff for this option.

Cathy Fitzpatrick, ARNP Retiring June 2011

Dear friends and colleagues,

I am writing to inform you that I will be leaving Pullman Family Medicine after 15 years of employment. My last day will be June 3, 2011. This has been a wonderful experience working with such a motivated and caring group of people. I have enjoyed the camaraderie of so many who have worked with me and supported my career as a Family Nurse Practitioner.

I am retiring to Hawaii to become “Grandma-nanny” for my youngest daughter in her developing family life.

The community of Pullman and the surrounding area will always be special to me. So many wonderful people have come through the doors of PFM! I have felt honored to be part of your health care team.

Over the years, I have felt close to many who have struggled with a variety of health issues. I will miss you and wish you the best.

The process of finding a replacement for my position is progressing with interviews and there should be a smooth transition to new relationships with the Pullman Family Medicine team.

Regards,

Cathy Fitzpatrick ARNP
